New and Incoming Student Athlete Medical Paperwork
Dear Student-Athlete,
Welcome to Wartburg College athletics. We look forward to your arrival on campus and assisting you in achieving athletic and academic excellence during your time at Wartburg. Prior to your arrival on campus in Waverly, you will be invited, via email, to activate your Healthy Roster account after June 1st. Healthy Roster will serve as your electronic medical record, accessed via an app on your smartphone or computer.
IF YOU HAVE NOT RECEIVED AN EMAIL INVITATION PLEASE EMAIL: Daniel.drees@wartburg.edu with your name, sport and birth date and we will get you set up.
Prior to your arrival in Waverly, you will need to complete the following list of documents within Healthy Roster and have a physical completed on or after June 1
st, 2024 (completed on the attached Wartburg Physical form).
To fill out the documents, download the app in the email, click on your name and then click on "Documents." Here you will find all the needed documents. If you cannot access the documents through the healthy roster app, delete the app and re-download it. Once completed the documents should appear.
Please complete in this order:
- Wartburg Physical Form – This must be completed on our form and must be signed off by a Medical Doctor (MD), Doctor of Osteopathy (DO), Nurse Practitioner (ARNP), or Physician Assistant (PA). Other providers will not be accepted per NCAA regulations. Completed on or after June 1st.
- Health History Form
- Wartburg Athletic Insurance Form
- Insurance Card Front and Back
- Sickle Cell Test/Labwork- (if sickle cell test results are on the physical form, you can skip this, must have results to participate) (Not required for Clay Target, Bowling, E-Sports, Cheer and Dance)
- Assumption of Risk 2024-25
- Concussion Awareness Form 2024-25
- Drug Testing Consent Form
- ADHD Medical Form
- Release of Health Information Form
